Proof of valid and active licensure to practice as a physician assistant or registered nurse and nurse practitioner in the State of California.
Letters of Recommendation: Please review the following guidelines to ensure the correct submission of your letters of recommendation.
One (1) letter of recommendation from the program director or current employing physician.
Two (2) letters of recommendation from recent supervising physicians.
One (1) letter of recommendation from a peer.
Legal employment in the State of California (possessing a valid passport).
Holding an unrestricted license to practice as an allied health professional in the state of California.
Possessing a current DEA license.
Certification by CAPA for the Controlled Substance Education Course.
Holding active CPR certification, BLS, and ACLS.
